FREE HEAD START Preschool-760-1776 Orchard View Community Education Center; 1765 Ada Avenue
This is a free preschool program for children 3 or 4 year old children who qualify
• 3 1⁄2 hour morning and afternoon half day sessions available Monday thru Friday
• All day sessions available Monday thru Friday
• Families qualify by income, special needs and family life experience factors
• Transportation may be provided and BREAKFAST, LUNCH and SNACKS provided

*All children must be 3 or 4 years old on or before September 1, 2017.

FREE GREAT START READINESS Preschool-760-1777 or 760-1810 Orchard View Early Elementary (Gustafson Campus), 2820 MacArthur Rd.
This is a free preschool program to 4 year old children who qualify
• All-day 6.5 hour sessions available Monday thru Thursday
• Families qualify by income and family life experience factors.
• Parent transportation is required
• Meals provided for All-day sessions

*All children must be 4 years old on or before September 1, 2017.

FREE EARLY HEAD START-through MAISD – 767-7253 Orchard View Community Education Center; 1765 Ada Avenue
• Servicing pregnant mothers and Newborn to age 3
• Home-based program.
• Year round weekly home visits.
• Qualify by income, special needs and family life experience factors.

CHILD CARE 760-1966
• Offers loving care in a nurturing environment at three locations. All are licensed by the State of Michigan.
o Orchard View Community Child Care: 2 weeks old to 6 years of age
o OVEE/Gustafson: 2 1⁄2 years and toilet trained through 6 years of age
o Cardinal Elementary: 6 to 14 years of age; Before and after school care

The following documents are required:

Birth Certificate: You must provide your child’s certified birth certificate. Certificates may be obtained from the county building (county in which your child was born). Required by Early Head Start, Head Start, and Great Start Readiness Program.
Health Forms: A green health form will be provided at the application appointment and is available at your doctor’s office. A physical examination is required. Immunizations are available for a fee at the Muskegon County Health Department, 209 E. Apple Avenue. Required by Early Head Start, Head Start, and Great Start Readiness Program.

Immunizations Record:  Official MCIR (Michigan Care Improvement Registry) from physician office.  Required for Head Start and Great Start Readiness Program.
Hearing and Vision: Tests are available at the Muskegon County Health Department. Call 724-6311 for an appointment. **Required for Head Start and recommended for Great Start Readiness Program.
Proof of Income: Proof of the last 12 months of income or last year’s income tax return or W-2 form. Required for Early Head Start, Head Start, Great Start Readiness Program.

Proof of Residency:  Copy of most recent utility bill including name and address.  Required for Head Start and Great Start Readiness Program.
Health Insurance Card: Medicaid, BCBS, etc. Required for Head Start.
Dental Examination: Required for Head Start.
